are sure that all GPs and all Q Codes are correct,

Check that you really do want to merge those 

Practices into this one . If so, press

You are trying to delete a relationship that is the result 

of a  merge .

unmerge the Practices, thereby resurrecting the most

really do want to permanently close this Practice. If
