Examples of valid Practice Codes are therefore: 212345, 222222 etc. (for Isle of Man, 

Jersey and Guernsey); or A12345, B77777, X80461, etc. (for English and Welsh HAs). 

NOTE 

It is important that the correct Practice Code is assigned to each Practice. This 

code may be required by other people outside your organisation in order to 

identify data, derived from your NHAIS System, for the Practice.

Practice Name 

Enter the name by which the new Practice is usually known (max. 50 characters). 

Completion of this field is mandatory. 

Responsible HA 

Enter the Q code of the Health Authority to which the new Practice will be responsible. 

(A Practice will always be responsible to only one HA.) Completion of this field is 

mandatory. 

Date registered 

Enter the date when this Practice was first registered at your HA. Completion of this 

field is mandatory. 

Date first registered 

Enter the date when this Practice was first registered at any HA. This date must be on 

or before the `Date registered'. Completion of this field is mandatory. 

Contract Type (G or P) 

Enter `G' if this Practice is operating under a GMS contract, or enter `P' in this field if 

the Practice is in the PMS scheme. 

Contract Start Date 

Enter the date to show when this Practice was, or will be operating within the contract 

type specified in the Contract Type field. 

NOTE 

For new responsible Practices, the completion of the Contract Type and Contract 

Start Date fields are mandatory. 

Date closed 

Only use this field if you are permanently closing an existing Practice. Entry of a date in 

this field when setting up a new PCO will result in a reject message being displayed. 

NOTE 

Once a Practice has been closed, no further changes can be made to any of the 

data held for that Practice.

The date entered here effectively becomes the end date for the latest live relationship for 

the Practice. See the heading  Closing a Practice  on page 13 16. 

Practice address 

Enter the main address of the Practice (max. 5 lines of 35 characters each). It is 

mandatory to enter address information in at least the first one of these fields. 

Postcode 

Enter the postcode for the main address of this Practice. This is a mandatory field.
