MESSAGE 

MEANING / ACTION 

DATE in use 

You have entered a Start Date for which relationship 

information is already held for this PCO. 

 Enter the 

correct Start Date.

DATE out of seq 

You have entered a Start Date on or before the start date 

of the most recent relationship already recorded for this 

PCO.  (If the previous relationship is one that was 

 merged  into another PCO, you will also get this 

message if the Start Date entered is not later than the 

End Date of the previous relationship.)  

Enter the 

correct Start Date.

Prac/Start conflict 

The entered Start Date must be later than any on record 

for the Practices entered.  It must also be later than any 

`end date' which may exist for the Practices due to them 

having been `merged out' at some time, or due to them 

having been within the last relationship of a PCO which 

has closed. 

PRACTICE missing 

You have either not entered any Practice Codes for a 

new PCO, or have removed all the Practice Codes for an 

existing PCO.  At least one Practice Code must be 

specified.  

Specify the Practice Code(s) for this PCO.

PRACTICE dup 

You have specified the same Practice Code twice.  

Remove one of the duplicated Practice Codes.

PRACTICE unknown 

You have specified a Practice Code for which no  core  

information has been specified via the PC screen.  

Either correct the Practice Code, or define the practice 

on the PC screen.

PRACTICE in use 

You have specified a Practice Code which is currently 

specified in another PCO. 

 Either correct the Practice 

Code, omit the practice from this PCO, or remove the 

practice from the other PCO.

NO GPs in Practice 

Any new practice added must contain at least one senior 

or single handed GP. This ensures that the DTS 

addresses are sent to QMAS with new practice data. 

Pracs   must merge all 

If you are attempting to  merge  other PCOs into this 

one, you must enter all the Practices from the other 

PCOs. 

ACTION missing 

You have changed the relationship information for this 

PCO, but have not specified an Action Code.  

If you 

wish to amend the relationships within this PCO, enter 

`A' in the Action field.

ACTION invalid 

Only `A' or `D' can be entered in the Action field.  

Enter either `A' or `D'.
