NOTE 

It is important that the correct National Code is assigned to each 

organisation.  This code may be required by other people outside your 

organisation in order to identify data, derived from your NHAIS System, 

for the PCO.

Date established 

Enter the date on which this PCO was first registered at any HA (in DD.MM.YYYY 

format).  Completion of this field is mandatory. 

Name 

Enter the name of this PCO (max. 50 characters).  Completion of this field is mandatory. 

Level 

Enter the level of the PCO (1   4).  Completion of this field is mandatory. 

Responsible HA 

Enter the Q code of the Health Authority to which the new PCO will be responsible.  (A 

PCO will always be responsible to only one HA.)  Completion of this field is mandatory. 

Date registered 

Enter the date when this PCO was registered at your HA (in DD.MM.YYYY format).  

This date can be the same as, or later than, the `Date established'.  Completion of this 

field is mandatory. 

Date closed 

Only use this field if you are permanently closing an existing PCO. Entry of a date in 

this field when setting up a new PCO will result in a reject message being displayed. 

NOTE 

  Once a PCO has been closed, no further changes can be made to any of the 

data for that PCO. 

The date entered here effectively becomes the end date for the latest live relationship for 

the PCO.  See the heading  Closing a PCO  on page 12 12. 

Contact Name 

Enter the name of the person who is the principal contact for this PCO (max. 30 

characters).  This field is optional. 

Address 

Enter the address of the principal contact for this PCO (max. 5 lines of 30 characters 

each).  It is mandatory to enter address information in at least the first one of these fields. 

Postcode

Enter the postcode of the principal contact for this PCO (optional). 

Normal STD code, Tel. no., Fax no.

Enter the STD code, telephone number and fax number of the principal contact for this 

PCO (optional). 

Other STD code, Tel. no., Fax no.

Enter the STD code, telephone number and fax number of one other contact for this PCO 

(optional).
