To enter all the details required for quarterly FP45 details, two pages will need to be 

displayed. The first page, page 4, allows for recovery of sick leave payment, maternity 

leave, vehicle running costs, Professional Defence Union Fees, and employer s National 

Insurance contributions screen used to enter quarterly FP45 details. 

The second page, page 5, is used for adjustments and general recovery amounts for 

quarterly FP45 returns.

trainee s GMC number/Q,start date,trainer s GP code.
