ASST CODE absent 

Assistant code must be present. 

CODE already exists 

Assistant code already exists for another doctor. 

CODE cannot change 

Cannot amend assistant code if details are on file already. 

ASST CODE invalid 

Must start with * and contain alphanumeric characters. 

ASST CODE too short 

Must contain at least two characters. 

SURNAME absent 

Surname must be present. 

SURNAME invalid 

Characters allowed are A Z . ,     and space. 

FORENAMES invalid 

Characters allowed in forenames are A Z . ,     and space. 

DOB missing 

Date of birth must be present. 

DOB invalid 

Must be a valid date of the form DD.MM.YY(YY) 

DOB too recent 

Year of birth must be more than 18 years ago. 

DOB too old 

Must not be older than 120. 

TITLE invalid 

Characters allowed in title are A Z . ,     and space. 

SEX invalid 

Sex must be F, M or space. 

ADDRESS invalid 

Characters allowed in address are 0 9 A Z . ,     / ( ) and 

space. 

PCODE but no Addr 

Postcode cannot be entered if address is not present. 

POSTCODE invalid 

Must be a valid postcode format e.g. EX17 6SE or SW9 

or EC2H 6LH. 

GMC CODE absent 

GMC No. must be present. 

GMC CODE invalid 

Must be 7 numbers. 

GMC CODE exists 

QUERY message.  Caution that GMC code exists for 

another GP. 

COUNTRY BIRTH absent 

Country of birth must be present. 

COUNTRY BIRTH invalid 

Must be a valid code as shown on the FC screen. 

FULL RATE absent 

Must be present for an eligible doctor. 

FULL RATE invalid 

Must be  Y  or  N  for an eligible doctor.
