The assistant's surgery address.  Up to three lines of thirty characters each may be 

entered. 

Postcode

Must be a valid postcode if entered. 

GMC No

.    

The assistant's General Medical Council number.  This field is mandatory and is used in 

the annual accumulation of superannuation data. 

Country of Birth

Mandatory field.  Country codes can be viewed on the FC screen. 

Delayed Retirement Payment   Full Rate 

This field is displayed for English sites.  A doctor in England is eligible to receive Full 

Rate Delayed Retirement / Golden Thanks Payments if he/she has a commitment greater 

than 24% and either: 

he/she is between the ages of 60 and 64 at the start of the current quarter, or 

his/her 60

th

 birthday occurs before the end of the current quarter . 

The Delayed Retirement Full Rate field must contain either `Y' or `N' for eligible 

doctors, otherwise it must be left blank. 

The Start of Quarter date and the doctor's age on that date are displayed immediately 

below this field. 

Golden Thanks Payment   Full Rate 

Golden Thanks Payment   Reduced Rate (Wales only)       

These two fields are displayed for Welsh sites.  A doctor in Wales is eligible to receive 

Reduced Rate Delayed Retirement / Golden Thanks Payments if he/she is an 

unrestricted doctor and either: 

he/she is between the ages of 55 and 60 at the start of the current quarter, or 

his/her 55

th

 birthday occurs before the end of the current quarter . 

The Golden Thanks Full Rate and Reduced Rate fields must contain either `Y' or `N' for 

eligible doctors, otherwise they must be left blank. 

The Start of Quarter date and the doctor's age on that date are displayed immediately 

below these fields.
