of the Quarter or more than 64 years at the start of the 

Quarter.

is eligible for Reduced Rate Golden Thanks payments, 

and the GP's age will now be either less than 55 years at 

the end of the Quarter or more than 64 years at the start of 

the Quarter.

that an English GP is now eligible for Delayed 

Retirement payments.

Must be numeric. 

TELNO invalid 

Must be numeric or in the form 999 9999 or 9999 9999 

FAXNO invalid 

Only numeric digits, SPACE characters and ` ' characters 

can be entered in this field. 

ADDRESS too short 

An e mail address must be at least 8 characters long. 

`@' REQUIRED 

An e mail address must contain the `@' character. 

`.' REQUIRED 

An e mail address must contain the `.' character. 

INVALID format 

The e mail address you have entered is in an invalid
