commencing ZZZ (other than ZZZ100   ZZZ999) are not 

allowed. 

SURNAME absent 

Surname must be present. 

SURNAME too short 

Surname must be at least two characters. 

SURNAME invalid 

Characters allowed are A Z     & and space. 

SURNAME invalid 

Must contain at least one alphabetic character. 

FORENAMES absent 

Forename(s) must be present. 

FORENAME invalid 

Characters allowed are A Z     & and space. 

FORENAME invalid 

Must contain at least one alphabetic character. 

DOB invalid 

Date of birth must be a valid date of the form 

dd.mm.yy(yy) 

DOB in future 

Must not be in the future. 

AGE less than 18 

Date of birth must be at least 18 years ago. 

DOB too old 

Must not be older than 120. 

DOB v DR Full 

The date of birth has been changed for an English GP 

who is eligible for Delayed Retirement payments, and the 

GP's age will now be either less than 60 years at the end 

of the Quarter or more than 64 years at the start of the 

Quarter. 

DOB v GT Full 

The date of birth has been changed for a Welsh GP who 

is eligible for Full Rate Golden Thanks payments, and the
