GD SCREEN

Understanding the Services Provided Screen  

DHA Codes

There must be at least one valid District Health Authority Code present and it must be 

entered as shown on the FD screen.  It is specifically used to show DHAs for Cervical 

Screening applications. 

MPA Code

 Mandatory field.  Must be a code that has been predefined to the system, i.e. showing on 

the FC screen. 

Appointed Locum?

 Enter 

Y

 in this field, if the GP is a locum. 

Exclude from ID 

Enter 

Y

 in this field, if a GP cannot have patients attached (e.g. Contraceptive Services 

only).  The field will automatically display N if left blank. 

Other HAs

This field can be used to enter up to twelve organisations with which the GP is also in 

contract. 

Special Notes 

This space is reserved for details of a GP, to be entered as up to four lines of text. The 

top line will be displayed on the second line of pages 1, 2 and 3 of the GD screen. 

Services 

The Services fields are reserved for information about services specified in a GP's 

contract.  They are all mandatory and must be entered for finance claims purposes. 

Restricted/Limited List

 : 

This field is used to enter specific details of the GP services available.  The possible 

valid codes are: 

R

GP offers restricted services 

L

GP s list is limited (e.g. to an institute) 

N

GP s list is normal 

Contraceptive List : 

This field specifies the type of contraceptive service offered by a GP and is entered by 

using one of the following codes: 

F

GP offers to all NHS patients 

R

GP offers to his GMS patients only 

N

GP offers no contraceptive services
