Forenames 

The GP's forename

must be entered and consist of at least one character. 

DoB 

The GP's date of birth.  Must be in the format dd.mm.yy(yy) 

Sex 

 Must

Title 

Space is available for text, but if left blank will automatically show: DR 

Qual.

GP qualifications can be entered, but this field is for reference purposes only. You can 

enter up to 50 characters in this field; 30 in the first line and 20 in the second. 

Address

The GP's surgery address   up to four lines of 30 characters may be entered. 

Postcode 

Must be a valid postcode if entered. 

Tel Exchange

Name of telephone exchange. 

STD Code

The GP's surgery telephone STD code. 

Tel Number

The GP's surgery telephone number. 

FAX Number

The GP's surgery fax number.  This optional field can hold both the Area Code and the 

Local Number for the GP s FAX number. If used, digits may be entered, separated by 

hyphens (" ") or spaces. 

E mail 

This field will either contain  *  to indicate that the GP has an e mail address or be blank  

to indicate  that the GP has no e mail address assigned.  You can assign an e mail 

address to a GP, or amend an existing e mail address, via a pop up box. 

The pop up box is accessed by positioning the cursor in this field, and then using the key 

control sequence defined on your system.  Normally, this will mean pressing the

 keys at the same time.  If the GP has no e mail address, you can enter it 

in the pop up box.  If the GP already has an e mail address, but the address in the pop up 

box is incorrect, you can amend it. 

Certain characters may be encoded during input.  The insert, delete, backspace, home, 

and end keys are available to make it easier to edit an address. 

To remove the pop up box, press 

.  Any changes you have made will be 

retained on the system.
