Either: 

1. 

Enter details for a new GP, by using a key of GD with a blank qualifier; or, 

2. 

View or amend details for an existing GP by entering the GP code or name in the 

qualifier. 

NOTE 

If  a name is used in the qualifier, use surname followed by a comma, then 

the first and second initials of forenames e.g. WILLIAMS, C B
